Title A study on implementing computerize physician order entry in minimizing medication error in outpatient department of a private hospital / Mok Wai Yee.
Abstract Computerized Physician Order Entry system allows consultants to prescribe patient’s medication electronically. In Hospital, Computerized Physician Order Entry System essentially eliminates the need for handwritten paper orders, achieves cost savings and increased efficiency. The purpose of this research paper is to study on the effectiveness on Implementing Computerize Physician Order Entry in Minimizing Medication Error in Outpatient Department of A Private Hospital. The study took place in a Tertiary Private hospital in Perak outpatients department which involved outpatient clinics and outpatient pharmacy department. The Study was comparing the effectiveness of the Computerized Physician Order Entry in minimizing Pharmacy intervention and Medication Errors during the 50% implementation as and Full implementation. This is a retrospective cohort study utilizing secondary data as the sample of the study. Secondary data that retrieved were categories in two cohorts, Cohort 1 and Cohort 2. Cohort 1 as 50% implementation stage and Cohort 2 as 100% full implementaion of Computerized Physician Order Entry System. Study period was from June 2013 to May 2018, a total of 60 months data were study. Sample size of the study were 306801 prescriptions from Cohort 1 and 296824 prescriptions was from Cohort 2. SPSS versions 22 were used as tool for analysis to test the hypothesis by using Descriptive statistical technique, Correlation and Paired T test and Bar Chart Results shown that after implementing 100% Computerized Physician Order Entry, there is significant reduction in Pharmacy interventions but no significant in minimizing Medication Error. The results also revealed that Computerized Physician Order Entry unable to minimized Errors in Dispensing stage but showed significant reduction of Medication Errors during Prescribing Stage. The main Medication Errors reported were due to Wrong Drugs, Wrong dose dispensed to patient and one incident of wrong label. Limitation of this research study was this study only took place in one hospital, results might not generalize to other hospital setting. Study period only 60 months, results might be different if longer time taken for the study.
