Title Factors influencing the retention of government sector pharmacists in rural and remote areas : the Sarawak finding / Kho Boon Phiaw
Abstract Geographical maldistribution of healthcare workers is a contemporary global issue, affecting both developed and developing countries. One pertinent reason contributing to this issue is that it is difficult to retain health care workers in rural and remote areas. Numerous policies had been implemented to solve or alleviate this issue, but the lack in consistently between strategies drawn and actual needs of the health care workers rendered most of the strategies unsustainable. Similarly in Sarawak, it is difficult to get government sector pharmacists serve longer in the rural and remote areas, which is vital to ensure accessibility and continuity of pharmaceutical care treatment for the population there. In order to guide further strategies, this research was carried out to determine the significant motivating factors that compel government sector pharmacists currently serving in rural and remote Sarawak to serve longer in those areas. The study is designed based on the principles of causal research, with the hypotheses built upon a conceptual framework that the retention of pharmacist in rural and remote Sarawak is caused by four distinct and independent factors: individual, work, local environment and transfer policy related. To test these hypotheses, a survey questionnaire was used, with items and constructs adopted and adapted from the literature.Validity and reliability were reasonably established by conducting a pilot test. The questionnaire was sent via email to 136 government sector pharmacists currently serving in 48 health facilities located in rural and remote Sarawak. 91 questionnaires were returned, yielding a response rate of 66.9%. The collected data were analysed using reliability analysis, descriptive analysis and correlational analysis. The majority of the respondents are young (mean 27.5 years old), female (73.6%), Chinese (83.5%) and unmarried (75.8%). The result revealed that generally, the majority of the pharmacists do not wish to retain long in rural and remote areas. Retention in those areas of was significantly influenced by individual related factors, environment related factors and transfer policy related factors (all p-value < 0.001). Work related factors did not have significant effect on retention in those areas (p-value = 0.132). Hence future strategies to enhance the retention of government sector pharmacists in rural and remote areas of Sarawak must be focused and based on these significant factors, in order to achieve optimal impact.
